Introduction: Diabetic nephropathy is characterized by a loss of glomerular basement membrane GBM ; integrity which is a major cause of end-stage renal failure. Heparanase HPSE ; , the -D-endoglycosidase that specifically degrades the heparan sulphate proteoglycans, plays a key role in this aberrant remodeling of the GBM and has been shown to be up-regulated by hyperglycemic conditions such as found in diabetic patients. We have previously reported that Defibrotide DF ; , a polydisperse oligonucleotide with antithrombotic, thrombolytic, anti-ischemic proprieties, suppressed in vitro both the expression of heparanase gene and heparanase enzymatic activity in multiple myeloma tumor cell lines. Consequently, we address the question whether DF is effective to regulate the expression and activity of HPSE in human microvascular endothelial cell HMEC ; and rat kidney epithelial cell lines NRK52E ; growing in hyperglycemia conditions. Methods: HMEC and NRK52E cells were growing under standard glucose 5mM ; and hyperglycemia conditions glucose 30mM ; for five days with and without DF at concentration of 50 and 200g ml ; . Heparanase gene expression was evaluated through real time polymerase chain reaction RT-PCR ; using cDNA prepared from those cells. Expression of heparanase was also evaluated by flow cytometric analysis using a monoclonal antibody against heparanase. Heparanase activity was measured on HMEC cellular extracts with a heparan-degrading enzymatic assay Takara Biomedicals, Shiga, Japan ; . Results: Our results showed a significant increase of HSPE gene expression in HMEC and NRK52E cells cultured in high glucose concentration compared with standard conditions p 0.01 ; . This result was confirmed by Immunostaining using Steptavidin-FITC two step methods. DF treatment at doses of 50 and 200g ml reversed the upregulation of HPSE gene expression induced by glucose restoring the normal levels. The HPSE activity was measured in HMEC cells treated with DF 50-150g ml ; or saline growing under hyperglycemia conditions. Our results demonstrated that glucose-induced an increase on HPSE enzymatic activity and the treatment with DF in dose dependent manner reduced HPSE enzymatic activity. Conclusion: Our data suggests that DF not only has an effect on the downregulation of HPSE gene expression, but also decreases its enzymatic activity in endothelial cells. Since HPSE is a critical factor in maintaining GBM integrity and is elevated in diabetic nephropathy, it should be considered for the management of this disease. Further studies on HMEC and NRK52E cells to better define this mechanism of action are warranted and anagrelide. A dual system will therefore be established: a "sentinel surveillance network" to monitor trends in the contexts of use, making use both of the existing arrangements and of ad hoc "sensors" TREND project ; and a system for collecting analyses of products in circulation SINTES project ; . Such a system will also make it possible to respond to the demands of the joint European action on synthetic drugs. EVALUATION OF PUBLIC POLICIES In the field of evaluation, the OFDT is charged with: defining an overall framework for the evaluation of public policies, developing methodologies and know-how based, where appropriate, on work in other countries and establishing and following to their conclusion evaluations of public actions and spot and short term studies falling within this general plan. ASSESSMENT Within its field of competence, the OFDT will respond to requests for assessment or intervention from the main French and international bodies. The OFDT could be called on as an expert by local or national players seeking validation of an observation method. It has become evident that concurrent treatment with multiple analgesics from different categories that operate through different mechanisms or at different sites provides improved relief with fewer side effects. This has led to the concept of multimodal analgesia, in which multiple agents from different analgesic categories are used concurrently.s" Together, the concepts of pre-emptive, continuous, and multimodal analgesia form the basis of contemporary comprehensive pain management strategies. ORIGINAL STUDY: 100 CONSECUTIVE PATIENTS Over the last 6 years we have applied a pain management regimen based on the concepts of pre-emptive, continuous, and multimodal analgesia to patients undergoing outpatient orthopedic surgery. We have studied the effects of this regimen on patients undergoing arthroscopically assisted ACLR as outpatients, using measures of pain, function, and patient satisfaction over the first 5 postoperative days.54 Before this study, we were not aware of any reports in the literature of studies of pain management after arthroscopically assisted ACLR in outpatients that incorporated a 5-day follow-up period. Basic Study Model In our original study, we evaluated 100 consecutive outpatient ACLR patients between November 1992 and June 1993. The age of the patients was 26.5 k 8.7 ; years. There were 54 men and 46 women. Total average operating room time was 128 minutes, average tourniquet time was 90 minutes, and average time in the recovery room was 142 minutes. Surgical Technique Arthroscopically assisted ACLR was performed using a bone-patellar tendon-bone autograft. The graft was harvested through an anterior vertical incision. The remainder of the procedure, including concomitant meniscectomy or meniscal repair if necessary, was performed arthroscopically. The grafts were positioned isometrically and fixed in place with interference fit screws adjacent to the bone blocks in both the femoral and tibial tunnels. The femoral graft fixation was done from "inside out." After the intra-articular surgery was completed, the defect in the patellar tendon where the graft had been harvested was closed with absorbable sutures. Two 3.2-mm hemovac drains were placed, one in the joint and the other in the subcutaneous space, before closing. A tournquet was used during the entire procedure and released after application of the dressings.
