Bjective: This study examined factors associated with hypochondriacal concerns in a military population, a population in which increased reporting of somatic and psychological symptoms has been observed. The aim of the present study was to determine what influence combat and related variables had on the development of hypochondriacal concerns. Method: Subjects were military personnel from Iowa who were on active duty at the time of the Persian Gulf War. Over 600 veterans took part in a study of symptoms and illnesses occurring in this population. Hypochondriacal beliefs and attitudes were assessed by the Whiteley Index and somatic symptoms by a factoranalytically derived measure. Potential predictors included demographic and military characteristics, personal and family history, personality dimensions, and environmental factors. Results: Hypochondriacal concerns were significantly associated with level of education, branch of the military, personal history of depression, number of prewar physical conditions, family history of functional syndromes and physical conditions, negative and positive temperament and disinhibition, military combat, level of military preparedness, social support, and perceived life stress. Somatic symptoms were associated with these same variables plus level of income, personal history of anxiety, family history of depression, deployment to the Gulf, and level of combat exposure. A regression model for hypochondriacal concerns R2 0.31 ; included number of prewar physical conditions, negative temperament, lack of social support, and perceived life stress. Conclusions: Combat did not contribute to hypochondriacal concerns 10 years after exposure, so it is unlikely that such concerns accounted for the increased symptom reporting observed in this population. Therefore, the second part of this study randomised 160 anaemic patients to darbepoetin alfa 3.0, 5.0, 7.0 or 9.0micrograms kg SC once every two weeks or epoetin 40, 000 IU SC once a week for 12 weeks the epoetin dose could be increased to 60, 000 IU once a week if response was inadequate ; . Reducing the frequency of darbepoetin alfa injections resulted in no apparent loss of efficacy, with over 54% of patients in all dose groups achieving a Hb response or correction [13, 14]. The third study was again conducted in anaemic patients receiving chemotherapy for solid tumours, but darbepoetin alfa was given once every three weeks. 177 patients were randomised to double-blind treatment with darbepoetin alfa 4.5, 6.75, 9.0 or 13.5micrograms kg SC or placebo once every three weeks for 12 weeks. Preliminary data for the lower three doses suggests a Hb response of 31 to 51% [15]. The fourth study was conducted in the absence of chemotherapy to treat the chronic anaemia associated with cancer itself. This open-label study randomised anaemic patients with non-myeloid malignancies mainly breast, prostate and lymphoid ; , who were not receiving chemotherapy, to darbepoetin alfa 0.5, 1.0, 2.25 or 4.5 micrograms kg SC once a week. Again, increasing doses corresponded to increasing efficacy, with over 66% of the 96 patients receiving 1.0, 2.25 or 4.5micrograms kg achieving a Hb response. Red blood cell transfusion rates also seemed to be dose dependent, with the lowest rate 7% ; seen in the 4.5micrograms kg group. This study is continuing to enrol patients to evaluate once every three or four week dosing [16-18]. Phase III study The phase III study randomised 320 anaemic patients Hb 11g dl ; receiving platinum-containing chemotherapy for lung cancer to double-blind darbepoetin alfa 2.25micrograms kg SC once a week or placebo for 12 weeks. Study drug was withheld if Hb concentrations were 14g dl in women or 15g dl in men, and doubled if Hb had increased by 1.0g dl at week 6. The primary endpoint was the proportion of patients requiring a red blood cell transfusion from week 5 to the end of the treatment phase week 12 ; . This was significantly reduced with darbepoetin alfa compared with placebo 21% vs. 51%, p 0.001 ; . A significant reduction was also seen in the proportion of patients needing a transfusion over the entire treatment period i.e. weeks 1 to 12 vs. 60%, p 0.001 ; . A haematopoietic response Hb increase 2g dl or level 12g dl ; was achieved in 66% of darbepoetin alfa patients and 24% of placebo patients p 0.001 ; [19, 20]. The phase III study also investigated the impact of darbepoetin alfa on health-related QOL, using the Functional Assessment of Cancer Therapy-Fatigue FACT-Fatigue ; Scale. This scale assesses patients' self-reported levels of fatigue. Abstracts and posters reporting this study, and pooled results from this study plus the phase II solid-tumour dose-response study, suggest darbepoetin alfa reduces fatigue [19-21].
