Riddelliine, senkirkine, monocrotaline, retronecine, heliotridine, jacoline, jacozine, jacobine, seneciphylline, and senecionine are but a few of the numerous PAs produced by a wide spectrum of plants. PA-producing plants e.g., tansy ragwort, coltsfoot, hound's tongue ; , especially Senecio species, have long been problematic in the western United States and are well known for livestock poisonings. Fluctuating levels of PA contamination in the consumer food supply, especially via certain herbal teas e.g., comfrey ; , honey Beales et al. 2004 ; , dairy products, beef, and grains, are a function of drought and harvest or foraging conditions, and therefore exhibit aperiodic cycles of high expression. Churchill County happens to be the center of honey production in Nevada Churchill Co. 2005; Michigan State University Extension 2002 honey is also produced in Cochise County. Honey has been a particular focus for PA contamination; levels can vary from hive to hive by two or more orders of magnitude within the same foraging location and by time of year. PA-producing plants are particularly prevalent in both counties, where they can contaminate the domestic food supply as weeds; some, such as comfrey, continue to be sold by certain vendors of nutritional supplements and health foods. Clinical Impact Atopic eczema is a common skin condition, affecting all age groups, but is more prevalent in children. Frequently associated with other allergic and atopic conditions such as asthma and allergic rhinitis, it can vary in severity. A range of symptoms occur, which can cause discomfort or severe distress, and disruption of normal activities. The mainstays of treatment are regular use of emollients and, when appropriate, short bursts of topical corticosteroids of moderate to high potency. Pimecrolimus is a macrolactam of the immunosuppressant class. It is a selective inhibitor of pro-inflammatory cytokines and mediators in T cells and mast cells. There are two fully published, randomised controlled trials in adults and five such studies in children. Two of the latter being in abstract form only and one not yet published. Pimecrolimus has demonstrated significant benefits over placebo in a wide range of symptom related end-points. One Phase II dose-finding study used betamethasone valerate 0.1% cream as an internal control, which showed significantly greater clinical benefits than pimecrolimus. The most common side effect in clinical trials was transient burning at the site of application. As with all new drugs long term effects remain to be seen. During analysis of the safety data submitted for the US licence, a small excess of infections was observed in patients receiving pimecrolimus for one year. Viral infections reported included herpes simplex and zoster. No reports of skin atrophy, as seen with corticosteroids, have been noted. Financial Issues and NHS Impact The manufacturer's proposed indication for pimecrolimus is for the short term treatment of the signs and symptoms of atopic eczema all severities ; , and for intermittent long term treatment to prevent progression to flares in patients aged three months or older.
