RNA 6000 chips coupled with the Agilent 2100 Bioanalyzer Agilent Technologies, Palo Alto, California ; , where 500 ng was used to determine the 18S: 28S rRNA ratio. All RNA used in the current study met the minimum requirement of at least a 1.8 ratio of 18S: 28S rRNA. The RNA aliquots were stored at 80C before use. Microarray analysis. Affymetrix chip processing was performed at the Duke University Genome Core Facility. In a concerted effort to minimize inherent variability, all chips were processed on the same day, using the same manufacturing lot of reagents and same Affymetrix scanner, and all had scaling factors within 10% of the group mean value. Statistical analysis. Raw average difference values provided by the Affymetrix analysis from each chip were used for all statistical analyses. Paired t tests and multidimensional scaling MDS ; of total gene expression profiles were performed using Partek software Partek, Inc., St. Charles, Missouri ; . In the simplest of terms, MDS is mapping of high-dimensional data 6, 800 genes per patient ; into low-dimensional space, such that the distances between points mapped in low-dimensional Euclidean space represent the similarities, dissimilarities, or distances of the original data. In this case, the distance between points in the images approximates a dissimilarity between points in highdimensional space. Herein, metric MDS was performed based on the Pearson dissimilarity defined as [1 r] 2, where r is the Pearson correlation ; and mapped into two-dimensional space initialized with points drawn randomly from a uniform distribution in the range of 1 to Figures are generated by MDS to minimize a measure of badness of fit, called stress, which depends on the point-to-point dissimilarities in the data compared with the distances in low-dimensional space. We present the results of a study into the effect of unilateral total hip arthroplasty for osteoarthritis on the golfing habits and performance of 750 patients. We contacted a cohort of 750 patients by postal questionnaire, enquiring into their pre-operative and post-operative participation with golf. We received 698 replies of which 66 were golfers. Harris Hip Scores are routinely prospectively acquired pre-operatively and at regular post-operative intervals for all patients undergoing hip arthroplasty at this centre. Comparisons of hip scores for golfers were consistently higher than for non-golfers at all intervals. There was no statistical difference in the number of games played, maximum shot distance, shot accuracy, handicap or ability to mobilize around the course when comparing pre- to post-operative performance in the golfing population. We conclude that total hip arthroplasty does not have a detrimental effect on golf participation or performance and would advise that patients need not be unduly concerned about the restrictions of this type of surgery on this pastime. Several low-prevalence countries have started to reduce the incidence of acute hepatitis B by targeted vaccination of high-risk groups. However, following the WHO advice of May 1992, most of the western countries have now implemented universal childhood immunisation programmes with generally good 85% ; and excellent 95% ; reported vaccination coverage. 19 Many countries, including most European 10 17 ; , have complemented the universal infant vaccination with a universal adolescent catch-up immunisation programme. 16 In Europe two countries have chosen for adolescent vaccination only i.e. Switzerland reported coverage ~60% ; and Slovenia reported coverage ~98% . Clinical studies indicate that a 2-dose regime of hepatitis B vaccination in combination with a stronger adjuvant ; also yield excellent sero-conversion rates in adults and adolescents.20, 21 A 2-dose regime is anticipated to increase cost-effectiveness and compliance rates of adolescent vaccination considerably. A cost-effectiveness calculation of only a universal 2-dose ; adolescent hepatitis B vaccination, or a universal infant vaccination accompanied by a catch-up adolescent programme is currently being performed for the Dutch situation by the RIVM. Universal childhood vaccination has been shown to be very effective in several countries. Taiwan was the first country that started mass vaccination in 1984. In diseases such as chronic renal failure, secondary hyperparathyroidism involves abnormalities in PTH secretion and synthesis and PT cell proliferation. Progress has been made in understanding how Ca2 , Pi, and vitamin D regulate the synthesis and secretion of PTH as well as the proliferation of the PT cells Fig. 3 ; . The multicenter trials, both completed and ongoing, provide cause for optimism, for it is apparent that sudden cardiac death can be mediated by prophylactic therapy. Individual trials focusing on a relatively homogeneous patient group have been crucial to demonstrating a favorable treatment effect. However, there are a few themes that appear to transcend individual studies and that may, perhaps, be important in the clinical application of the results of the trials and in the direction of future clinical research. These general themes deserve special emphasis. First, the prophylactic therapies best substantiated by scientific data are substrate-oriented therapies rather than antiarrhythmic therapies per se. We would include in this group of well-substantiated therapies the use of ; 3-blockers in the post-MI patient population as well as the use of angiotensin-converting enzyme inhibitors in some subsets of patients with congestive heart failure. We would also include the use of revascularization in patients with diffuse coronary disease and left ventricular dysfunction in the group of effective therapies, although this treatment measure has not been tested as rigorously as the use of ; 3-blockers post-MI or angiotensin-converting enzyme inhibitors in congestive heart failure. Each of these therapies ameliorates the underlying cardiac pathology. Arrhythmia-focused therapy has failed when employed prophylactically. Whether the morbidity, mortality, and expense of device-based therapy will fare better than treatment with antiarrhythmic drugs remains to be seen. Second, the mechanisms of action of the effective therapies are poorly understood. It is not known whether the benefits of p-blocking agents are due to their anti-ischemic or antiarrhythmic properties. Nor is the precise mechanism known by which angiotensinconverting enzyme inhibition prevents sudden death. These agents are primarily known for their hemodynamic effects but also have anti-ischemic properties. Treatment of congestive heart failure with angiotensinconverting enzyme inhibitors also influences the autonomic nervous system, which may be important in arrhythmia prevention. The antiarrhythmic agent that currently holds the most promise for sudden death prophylaxis, amiodarone, has very complex cardiac effects. It has a favorable hemodynamic profile in heart failure and was first developed as an antianginal drug. If this drug is proven to be effective for sudden death. A total of 12 subjects 6 women and 6 men; 1840 years ; participated in the study. Exclusion criteria were: regular use of medications; abuse of alcoholic beverages; symptoms of significant illness within 3 months before the study period; history of gastrointestinal, liver or kidney disease potentially interfering with absorption, metabolism or excretion of drugs; history of CNS disorders; allergy or hypersensitivity to the study drug; antibiotic treatment within 3 months before the study period; and pregnancy. Written informed consent was obtained from all subjects prior to the study. The study was approved by the Ethics Committee of the Karolinska Institute, Stockholm, Sweden, and the Medical Products Agency, Uppsala, Sweden and cytoxan. The study population consisted of seven male and eight female outpatients, mean age 59 years range 3678 years ; , who had stable 6 months ; , chronic class III and IV heart failure by New York Heart Association NYHA ; criteria. All patients received digitalis and diuretics for at least 1 year. Ten patients had valvular heart disease one with mitral and three with aortic valvular incompetence, three with combined mitral and aortic regurgitation, and three with prosthetic mitral valve replacement ; , four with primary myocardial disease, and one with ischemic heart disease and documented myocardial infarction. Seven of the patients had chronic atrial fibrillation; the remainder were in sinus rhythm. All patients had radiographic cardiomegaly cardiothoracic ratio 50% ; .
