NITROGLYCERIN IN HEART FAILURE Flaim etal. nificant effect in the heart failure group at rest. During exercise, nitroglycerin significantly increased blood flow and reduced vascular resistance to the spleen in the heart failure group. Data for the distribution of total systemic cardiac output at rest and during exercise in sham and heart failure groups during saline and nitroglycerin infusion are presented in Table 5. In the sham group, nitroglycerin slightly increased the percent of cardiac output distributed to the renal circulation in resting animals and slightly decreased that going to the gastrointestinal circulation. During exercise, nitroglycerin increased cardiac output percent in the skeletal muscle bed and reduced that in the gastrointestinal bed. In the heart failure group, the percent of cardiac output going to the skin and gastrointestinal beds was reduced during saline infusion at rest, whereas that going to the coronary circulation was increased. During nitroglycerin infusion at rest, the gastrointestinal increment remained reduced compared to sham. During exercise, the percent of systemic cardiac output going to the brain and heart was increased and that going to the kidney and spleen was reduced during saline compared to sham. During nitroglycerin and exercise, there was no significant difference between the heart failure and sham groups. Nitroglycerin had no effect on the heart failure group at rest but significantly increased the percent of systemic cardiac output going to the renal circulation during exercise. Discussion The results of these studies indicate that nitroglycerin preferentially increased blood flow to the renal, splanchnic, and cutaneous circulations of rats with heart failure during exercise without significantly and consistently enhancing blood flow to the skeletal muscle bed. The data explain earlier observations that this vasodilator can simultaneously increase cardiac output without increasing systemic oxygen consumption Rubin et al., 1977; Moskowitz et al., 1979; Franciosa and Conn, 1979 ; , observations which lead to the suggestion that skeletal muscle nutritional blood flow may not be enhanced during acute vasodilator therapy with nitroglycerin for heart failure Zens et al., 1979 ; . In the present study, acute nitroglycerin infusion caused a nonsignificant increase in cardiac output in rats with heart failure, both at rest and during exercise. There were also slight but nonsignificant reductions in left ventricular end diastolic pressure in the group with heart failure both at rest and during exercise with nitroglycerin. Nitroglycerin had little effect on heart rate but significantly increased stroke volume in the animals with heart failure at rest. The major hemodynamic effect of nitroglycerin in the group with heart failure at rest was the fall in systemic vascular resistance. This reduction in afterload is most likely attributable to the combined effect of the drug in the renal.

Synopsis The scientific committee of the European Medicines Agency has issued a positive opinion recommending approval of adalimumab HumiraTM ; for the first-line treatment of moderate to severe early rheumatoid arthritis RA ; and for the treatment of psoriatic arthritis. The positive opinion for RA is based on data from the two-year, phase III PREMIER study showing that methotrexate-naive patients with early RA, experienced significant improvement in signs and symptoms while taking Humira in combination with methotrexate MTX ; , compared with those on MTX alone 62% on combination therapy achieved ACR 50 at 1 year compared to 46% in the MTX group and corresponding figures at two years were 59% and 43% ; . ACR 50 indicates a 50% or greater improvement in tender and swollen joint count and other clinical measures. The positive opinion for psoriatic arthritis is based upon two placebo-controlled studies, including data from the Adalimumab Effectiveness in Psoriatic Arthritis Trial ADEPT ; , a phase III trial showing patients on Humira achieved significant improvement in both arthritic and psoriatic signs and symptoms. Humira is currently indicated for the treatment of moderate to severe active RA in adults when the response to DMARDs has been inadequate. The EC is expected to issue an authorisation decision in approximately 90 days.

1 2 3 Because lavage requires general anesthesia, the risks should be comparable with the general experience with low risk surgical procedures. Deliberations on the justification of the procedure should consider the age and health of the patient relative to the possible deleterious effects of the radiation exposure. Contamination that carries a high risk of early radiation effects should be weighed against the risk of intervention. In general, though, the potential effects from internal contamination will occur 10, 20, or more years in the future while the risk of lavage is immediate. Elderly or chronically ill individuals may, therefore, experience relatively little benefit from such intervention procedures. In a review of clinical and experimental uses of bronchopulmonary lavage, Muggenburg and Jones 1971 ; noted that a single death was recorded after 204 lavages in 70 patients who had various types of severe obstructive lung disease. In dogs, two deaths were recorded after 420 lavages in 149 animals. An update on humans cites one death after 258 lavages in 112 persons Muggenburg et al., 1976a ; . Nolibe et al. 1976 ; report five deaths in baboons in the course of 800 lavages. This overall experience suggests a possible mortality rate of approximately 0.5 % for each lavage procedure. Since the human cases were complicated by the presence of debilitating pulmonary disorders, the estimated risk is probably greater than is to be expected in otherwise healthy individuals. Possible use of this technique in man requires a careful risk-benefit assessment. The risk of the procedure lies with both the administration of a general anesthetic and any mortality or morbidity that could be associated with the procedure. Muggenburg et al. 1972 ; found that dogs serially sacrificed after lavage had no physiologic or pathologic changes one week after the procedure. Early physiologic alterations resolved within 24 h and only vestiges of tissue reaction were found by light microscopy at 48 h. After three lavages of the man contaminated with 239 Pu there was neither evidence of impaired lung function nor signs of adverse reactions 24 h after the last procedure. Studies of surfactant lipids in the lungs of dogs show that the lipids removed by lavage are replaced in about 5 h Henderson et al., 1975 ; . Muggenburg et al. 1976b ; have shown in dogs that multiple lung lavages carry little biomedical risk and that the primary risk is associated with general anesthesia and hydrea.

302 | Season 7 I was delighted to see the way they captured the COPS look and feel complete with the "Bad Boys" theme, blurred out faces, bleeped out profanities, and, best of all, the requisite crack house filled with belligerent idiots and a topless woman. Director Michael Watkins fully captured the style of the COPS huffing and hoofing camera crew and creatively used flashlights of all sizes, streetlights, and the strobe of police cars to light his scenes. While an hour of racking in and out of focus with a jostling camera can be tiring to watch, it certainly was true to form. I think my favorite camera moment was the inside of the car shot as the squad car was flipped and everything inside flew around to end the teaser. Watkins also managed to bring the right mood out of the actors with the COPS camera around. Officers like Wetzel, trying to sound good and profound, wax poetic about their jobs and cast nervous glances towards the lens when they are afraid they might not come off well. Mulder starts playing directly into the camera himself, filling the audience in during the evening with his own special brand of narration. However, I thought the smirking he was doing, most notably in the Steve and Edy scenes was a little too much for my taste. I had a hard time telling if it was Mulder or Duchovny that found the goings on so amusing, especially since they were doing so many long and complex takes. And Scully, well, Scully true to form hates the camera and spends as much time as possible ducking out of its way, turning her head, covering it with her hand, and avoiding it. I really enjoyed the looks and frustration Gillian Anderson brought to this throughout the episode from that initial puzzling when she first sees the camera to the phony sweet smile at "the FBI has nothing to hide" to her final exasperated door slam complete with "I hate you guys". Most may have missed it, but Scully was actually attacked by the fear monster in this episode as well. Her intense mortal fear of being caught on television cameras with Mulder spouting off theories about werewolves and the like came to fruition. Only the fear monster chose not to kill her, because when Mulder talks the paranormal talk for the cameras Scully probably actually wants to die so letting her live is much more painful. So while Mulder happily chronicles for the camera the nature of paranormal crimes she's reduced to croaking out the occasional "oh yeah" in response. It's when Scully gets in her element, the autopsy bay, that she also begins to show off for the camera. She smugly informs the nurse that she's about to put the ghost murderer story to rest and goes about oh-soprofessionally snapping photos to punctuate her statements. It makes it all the more funny when a casual reference to the hanta virus strikes the nurse down at the mere mention and leaves Scully flustered and frustrated. "X-Cops", as fun as it was for a change of pace episode experiment, was just at the edge of how much of this style one could actually take before the gimmick got old. Actually, having to watch the herd of cops run up the dark street one more time made me wish for more meat and less chaos. As an experiment I'd rate it as a success, but one with a story stretched too thin for its own good. In the end I wondered how many of Mulder and Scully's cases are just like this: a lot of scrambling around and running in the dark with empty hands come daylight. Random Musings Looks like FOX was a little bit paranoid that their viewers would not know what the hell they were watching, so in addition to slapping the name in an unnecessary viewer discretion statement they insisted on making that little floating logo in the corner even more obtrusive and bright. Though I did really like that red and blue strobe logo fadeout they did between acts. Kudos to the set decorating team for capturing the truly trashy so wonderfully. From bathroom fixtures in the yard to the magic- motion Jesus photo to the plastic bead door, everywhere the camera lingered tackiness abounded. They even managed to have cats eating at the table. You can't ask for much more than that. I enjoyed the way Doctor's curiosity got the better of Scully and she was forced to pop out from behind her hiding place at the ambulance door to examine Wetzel's "bite" before quickly retreating again. Speaking of Dr. Scully, why in the world does one shout out "call 911" in a hospital? Is it so paramedics can arrive and take the person next door? Our Little Sailor: Scully expresses her dissatisfaction with the camera crew "Mulder, what the hell is going on here?" and her frustration with the hanta virus incident "the hell I wasn't afraid, Mulder". I'm a little disappointed in Vince Gilligan going for the obvious joke as Wetzel tries to relate to Mulder how horrible it is if other cops think you're crazy. Oh, we get it. Fun moment: Scully suddenly realizing Mulder looks pretty bad since his werewolf bite turned out to be bug stings and trying to protect him by acknowledging the camera for the first time to explain that it was a completely valid conclusion that "anybody would have made". I must admit, I never thought I'd actually hear the words "skanky ass" on the X-Files, let alone Mulder and Scully refereeing a fight between two old dra and hydromorphone.

In spite of rehabilitation services, treatment, legal rights Americans with Disabilities Act ; , and charitable approaches, public perceptions of people with disabilities are troubling. Consider that people with disabilities. continue to be labeled at the drop of a hat. Usually these labels are medical in nature and create huge cultural stereotypes. are still readily institutionalized. Although many institutions have shifted from large, gothic settings to smaller, group facilities, they are still institutions in format.
