Ejection fraction is the pivotal index of systolic function in clinical practice. Although the clinical utility of ejection fraction is well established, this index may be unreliable because of inaccuracy in the estimation of LV volumes and, most importantly, its load dependence.31 Ejection fraction is particularly modified by load when contractility is impaired, 18 an issue that limits its applicability under extreme or changing load conditions. In these situations, Doppler-derived EIVPDs could provide a more reliable measurement of systolic chamber function. Because the Doppler signal is less sensitive to noise than cross-sectional echocardiography, EIVPDs are obtainable even in patients in whom poor endocardial definition precludes an accurate estimation of LV volumes. The present study demonstrates that the measurements of EIVPDs are relatively accurate as long as scan line misalignment is 30 and displacement from the LVOT center is 0.5 mm. Because CDMM acquisition is always guided by 2D echocardiography, these constraints generally can be met during clinical examination, as suggested by our reproducibility results.
It is therefore possible that high dose intravenous immunoglobulin after the plasma exchange may have been useful. In a report from 2004, Smith and colleagues analyzed the two dual infection cases mentioned above plus a third man with apparent secondary infection wild-type followed by drugresistant virus ; . INTRODUCTION By definition, celebrities are newsworthy. Their star qualities draw the news and entertainment media to report not only on issues directly relevant to their initial cause of celebrity but on virtually any public utterance they make. Whole television series have been built around peering into the private lives of celebrities. Television lifestyle programmes on anything from cooking, home renovation and health routinely incorporate celebrity profiles. For decades, television interview programmes have in turn made vicarious stars out of those who interview prominent people. Even mere public sightings of famous people are reported regularly in the media. When celebrities die, are ill, change their weight or some other aspect of lifestyle, or undertake medical procedures, health issues can be given substantial publicity as reporters seek to provide background on what the celebrity is experiencing. Media commentators often draw evaluative, moral lessons about these experiences. Organized health promotion and advocacy campaigns have long understood that by engaging a celebrity with a health issue or capitalizing on the interest generated by news of celebrity illnesses, coverage of the issue de jour can be increased to levels that would otherwise require stratospheric campaign budgets Baker et al., 1992; Chapman and Lupton, 1994a ; . It's an earnings driver, " said David Moskowitz, an industry analyst at Friedman Billings Ramsey. "At this point, it's a show-me story. It's very unfortunate they haven't been able to supply the product. They really need to show the marketplace they can supply it in sufficient quantities by the end of the year." One former Wyeth employee maintains shareholders have already suffered. In a whistleblower lawsuit filed last December in U.S. District Court in Greensboro, N.C., Mark Livingston, a quality-control expert, charged Wyeth managers at the Sanford, N.C., plant cut corners to boost Prevnar production. He also charged that his suggestions for meeting FDA standards were deliberately ignored and that he was wrongly fired during 2002. The company's practices, according to the lawsuit, amounted to fraud and violated the Sarbanes- Oxley Act, which was designed to improve corporate governance and financial disclosure to shareholders. For instance, filings with the Securities and Exchange Commission didn't mention a negative FDA inspection report at the Sanford plant during September 2003. MISREPRESENTATIONS "They misrepresented the real state of manufacturing, " said Joanne Royce, an attorney with the Government Accountability Project, which represents Livingston.
