10.1 Disqualification of Results in Event During which an AntiDoping Rule Violation Occurs An anti-doping rule violation occurring during or in connection with an Event may, upon the decision of the ruling body of the Event, lead to Disqualification of all of the Athlete's individual results obtained in that Event with all consequences, including forfeiture of all medals, points and prizes, except as provided in Article 10.1.1. [Comment: Whereas Article 9 Automatic Disqualification of Individual Results ; Disqualifies the result in a single Competition in which the Athlete tested positive e.g., the 100 meter backstroke ; , this Article may lead to Disqualification of all results in all races during the Event e.g., the FINA World Championships ; . Factors to be included in considering whether to Disqualify other results in an Event might include, for example, the severity of the Athlete's anti-doping rule violation and whether the Athlete tested negative in the other Competitions.] 10.1.1 If the Athlete establishes that he or she bears No Fault or Negligence for the violation, the Athlete's individual results in the other Competitions shall not be Disqualified unless the Athlete's results in Competitions other than the Competition in which the anti-doping rule violation occurred were likely to have been affected by the Athlete's anti-doping rule violation. 10.2 Imposition of Ineligibility for Prohibited Substances and Prohibited Methods Except for the specified substances identified in Article 10.3, the period of Ineligibility imposed for a violation of Articles 2.1 presence of Prohibited Substance or its Metabolites or Markers ; , 2.2 Use or Attempted Use of.

SyncroDoseTM Drug Delivery Technology Penwest developed the patented SyncroDose technology to deliver drugs chronotherapeutically in the body or to deliver a drug to a specific site in the gastrointestinal tract. By administering a drug at the optimal time after ingestion, or to a specific site, SyncroDose has the potential to improve the therapeutic benefit of drugs and reduce the number of required daily doses. We believe that there are several disease states that can benefit from chronotherapeutic delivery. These include: arthritis, cardiovascular disorders, asthma, neurological disorders and GI cancers. Gastroretentive Technology Penwest developed the gastroretentive technology to deliver certain drugs that are absorbed in the stomach and certain portions and reyataz.

There is also no reference to the issue within social service progress notes that were done upon admission. Physician's order sheets from the same time frame include prescriptions for behavioral and sleep aid medicines, which were signed by the same attending physician. A progress note from the attending physician, written just ten days after the psychiatrist saw the resident, stated that she met with the resident to review medications and complaints of drooling and weakness. She made no changes to the medications and made no mention of the psychiatrist's earlier evaluation. On the issue of administering medications as prescribed, only one instance of a possible error stood out according to the physician's order sheets, nursing progress notes and medicine administration records that were reviewed. This occurred on July 12th when the sleep aid, Restoril, was given at 1: 30 a.m. and not before midnight as reportedly directed. Further scrutiny of the record however revealed that the order had been changed several times. Orders upon admission stated that Restoril was to be given at the hour of sleep, about 8 p.m. On January 25th, the attending physician allowed a second dose as needed PRN ; but to be given at the same time. In March, the physician's order sheet directed that the PRN dose was to be given before midnight. In May, the scheduled Restoril order was discontinued, and the PRN order was left without a specific administration time. This change was carried through the end of July, just before the scheduled hourof-sleep dose was resumed in August. The attending physician and the facility's reviewing nurse signed all of these order sheets. There were no administration errors noted regarding Zyprexa, the only other prescribed behavioral medication. Regarding open records at the nurses' desk, the HRA visited the Alzheimer Unit and saw no exposed confidential materials. Resident charts were situated well behind the desk, and although full names were written on them, the names could not be clearly read--at least not by us. When we asked if there was a bowel movement registry, we were shown a notebook that was kept at the desk but under the counter, away from general sight. The Director of Clinical Services said that confidentiality training is conducted with employees at orientation and annually thereafter.

Relapse was defined by Southwest Oncology Group criteria as a greater than 25% increase in M protein from baseline levels, reappearance of the M paraprotein that had disappeared, or definite increase in the size and number of lytic bone lesions recognized on radiographs. Patients with renal insufficiency related to MM, including those undergoing dialysis, were eligible. Exclusion criteria included grade 3 or higher peripheral neuropathy, mucosal or internal bleeding, absolute neutrophil count lower than 0.75 109 L, pregnancy or lactation, and women of childbearing potential who were not using contraception. SAFETY PARAMETERS Before enrollment, at each 2-week and 4-week visit, and either at completion or at discontinuation of the study, evaluation of each patient included the following: medical history and physical examination, including assessment of peripheral neuropathy and measurement of vital signs; query regarding adverse events and concomitant medication use; and clinical laboratory testing, including blood chemistry and hematology. The severity of adverse events, including any clinical and laboratory abnormalities, was evaluated using CTC from the National Cancer Institute. EFFICACY PARAMETERS Response to treatment was assessed using M protein quantification in serum and a 24-hour urine collection at baseline, at weeks 8 and 12, and monthly or at termination of treatment, with highest reduction in M protein based on Southwest Oncology Group criteria ; used to define response Table 1 ; . Responses were confirmed by repeated paraprotein measurement up to 6 weeks apart, with additional BM and radiological evaluation as indicated clinically. Bone marrow aspirations and or biopsy were performed at 3, 6, and 12 months. A skeletal scan was obtained in patients with abnormal findings before treatment or if they had symptoms. SURROGATE MARKERS Serum levels of IL-2, IL-6, IFN-, sICAM-1, and TNF- as well as serum and plasma levels of VEGF were measured by enzyme-linked immunosorbent assay, using commercially available kits and according to the instructions of the manufacturer R & D Systems, Minneapolis, Minn ; . All standard curves were linear, all samples were run-in duplicates, and human IgG did not interfere with these assays. For IL-6, VEGF, TNF-, IL-2, IFN-, and sICAM-1 assays, the minimum detectable cytokine levels were 0.70 pg.

In general, responses to surgery involve changes in the distribution and function of members of the leukocyte family. These changes include early systemic leukocytosis characterized by an increase of neutrophils and monocytes and a decrease of lymphocytes and eosinophils Salo 1992 ; . In addition to the postoperative decrease of the total lymphocyte count, the distribution of lymphocytes has been either altered, demonstrating a fall in T-cell levels and a decrease in the ratio of CD4 to CD8 cells, or stable Gupta 1987, Ryhnen et al. 1991 ; . Lymphocyte functions have also been altered after surgical stress, including defects in antigen recognition, a proliferative response and a decrease in antibody production Brandt 2001 ; . Macrophages have shown increased phagocytosis and microbicidal activity but decreased class II major histocompatibility complex MHC ; molecule expression after surgery Neoptolemos et al. 1985, Ryhnen et al. 1991 ; . Studies on monocytes from injured patients have identified an association between reduced postoperative HLA-DR expression and subsequent infection and mortality Cabi et al. 1992 ; . Studies on the effect of surgery on the postoperative neutrophil response in human patients have been gathered in Table 4. Some of these studies have also explored monocyte functions.
