Venous thrombosis disease is an important cause of morbidity and mortality. Incidence is low in young people but increases with age to 1% per year in the elderly and increases when risk factors are associated. In a small proportion of cases, venous thrombosis leads to pulmonary embolism, which can be fatal. Each year millions of patients will die from blood clots that obstruct blood flow to their lungs pulmonary embolism ; . The tragedy is that most of these problems could be avoided by simple, cost-effective measures. Use of modern methods of DVT prophylaxis will reduce the incidence of DVT during the postoperative period by two-thirds and will prevent death from pulmonary embolism in 1 patient out of every 200 major operations.

Disease progression. For the treatment of AS, traditional DMARDs are not effective, and therefore, clinicians need to consider earlier use of TNF antagonists, which reduce the signs and symptoms of the disease. Preliminary data suggest that TNF antagonist therapy may result in structural modification and halt disease progression in AS. In patients with PsA, optimal doses of MTX or sulfasalazine can control joint symptoms, while TNF antagonists are useful in controlling both skin and joint symptoms and can halt disease progression. It is worth noting that combination therapies require close monitoring and that ongoing treatment with any regimen requires objective measurements serial radiographs, C-reactive protein, etc ; to be sure that optimal outcomes are obtained. The place of biologic therapy in the paradigm of inflammatory arthritis treatment is still evolving. Guidelines exist for use of these drugs in various inflammatory arthritides, 15, 18, 88, with most having been developed in Europe. Moreover, the data available for new treatment options are accumulating continuously and must be assimilated before treatment guidelines can be updated; therefore, clinicians need to make treatment decisions based on the individual patient. Nonetheless, the treatment goal remains remission or very low disease activity, and as such, the use of biologic agents represents a remarkable step forward in the effective management of these conditions.

DIAGNOSTIC TESTING Currently, there are several popular methods for detecting the presence of H. pylori infection, each having its own advantages, disadvantages, and limitations. Alternatively, simple breath tests, serology, and stool assays are sometimes used, and trials investigating PCR amplification of saliva, feces, and dental plaque to detect the presence of H. pylori are ongoing.49 Histology Histologic evaluation has traditionally been the goldstandard method for diagnosing H. pylori infection. The disadvantage of this technique is the need for endoscopy to obtain tissue. Limitations also arise at times because of an inadequate number of biopsy specimens obtained or failure to obtain specimens from different areas of the stomach.50 In some cases, different staining techniques may be necessary, which can involve longer processing times and higher costs. However, histologic sampling does allow for definitive diagnosis of infection, as well as of the degree of inflammation or metaplasia and the presence absence of MALT lymphoma or other gastric cancers in high-risk patients. Culture Because H. pylori is difficult to grow on culture.

Sulfasalazine has been shown to be effective in patients with chronic reactive arthritis.22 Intra-articular corticosteroid injections can be effective in controlling disease in individual joints. Antibiotic therapy has not been effective in several trials.23, 24 An association between certain spondyloarthropathies reactive arthritis, psoriatic arthritis, undifferentiated spondyloarthropathy ; and human immunodeficiency virus infection has been noted, particularly in subSaharan Africa.25 The patients have tended to have more severe disease, and the increased prevalence is thought to be due to poor availability of effective antiretroviral therapy. Psoriatic Arthritis Psoriasis affects up to 1 percent of the general U.S. population, and psoriatic arthritis has been reported in as many as 20 percent of patients with psoriasis. Treatment and the drugs used in treatment have many side effects. Although most of these drugs do not treat the disease but act to suppress the symptoms, prevention of functional loss and improvement of quality of life is provided with this therapy. Since the disease is progressive and can persist in adulthood, long term combined drug therapy may be necessary in some subtypes of the disease3, 4, 5. That is why determination and follow up of side effects of the drugs is important in terms of morbidity and mortality. Duration and dosage of drug therapy, combined therapy, drug interactions and metabolism play an important role in determining the side effect profile6, 7. Step by step treatment is recently preferred starting with NSAIDs and ending with the DMARDs as needed. Even though the nonsteroid anti-inflammatory drugs NSAIDs ; are still the first-line agents in initiation and maintenance therapy, most of the patients need second line agents and corticosteroids 3 ; . Most of these second-line agents, named Disease-Modifying Anti-rheumatic Drugs DMARDs ; , delay the radiological progression of the disease. Among these drugs, methotrexate, sulfasalazine and etanercept are found to be effective in treatment of JIA, as a result of double-blind placebo-controlled studies 4 ; . The choice of treatment depends on the weight of the patient, speed of effectiveness and toxicity. Most commonly preferred drugs are methotrexate and sulfasalazine in this group. Methotrexate, sulfasalazine and corticosteroids among DMARD, have many systemic side effects4, 5, 8, 9. Gastrointestinal system GIS ; has great importance in terms of primary side effects and metabolism. In chronic diseases in which long term therapy is needed, most important factor in determining adjustment to treatment is the type and severity of GIS side effects. In our study, we aimed to determine the side effects and long-term reliability of mono-therapy and combined therapy of DMARDs methotrexate, sulfasalazine ; -except NSAIDs and corticosteroids. Features of GIS side effects are evaluated with laboratory and radiological work-up and the difference of side effects between single and combined drug therapies is investigated. Patients and Methods Fifty out of 119 patients with JIA, followed-up in our Rheumatology out-patient clinic between the years 1996-2006 were investigated retrospectively in the study. The inclusion criteria were; the age at disease onset less than 16, coming to follow-up at 3 months interval at least for 2 years, receiving sulfasalazine, methotrexate or steroid therapy at least one year and receiving at least one of them. None of the patients received NSAID. Patients with systemic JIA that received pulse steroid 30 mg kg day ; and cases with other chronic disease were excluded from the study. The patients and their families were informed that these data will be used in a study and written informed consents were taken. ILAR diagnostic criteria were used in grouping the patients 10 ; . Sixteen 32% ; of the patients were diagnosed as oligoarticular JIA, 15 30% ; as polyarticular RF - ; JIA, 12 as 24% ; systemic onset JIA, 6 as 12% ; enthesitis-related arthritis and 1 as 2% ; was psoriatic arthritis. Methotrexate was used as 10-20 mg m2 week, salazopyrine 50 mg kg day, methyl prednisolone 0.5-2mg kg day in our patients the preferred dose was 10mg day ; . Steroid therapy was used as a bridge therapy intermittently in patients with systemic JIA, poliarticular JIA and extended oligoarticular JIA. Among these patients, Full blood count, acute phase reactants, biochemical parameters, PT, aPTT and occult blood in feces were checked in every visit. Together with occult blood in feces, only aspartate aminotransferase AST ; and alanine aminotransferase ALT ; levels were compared in terms of GIS side effects. Mutants of Drosophila melanogaster, with suspected repair deficiencies, were analyzed for their capacity to repair damage induced by X-rays and UV radiation. Analysis was performed on cell cultures derived from embryos of homozygous mutant stocks. Postreplication repair following UV radiation has been analyzed in mutant stocks derived from a total of ten complementation groups. Cultures were irradiated, pulse-labeled, and incubated in the dark prior to analysis by alkaline sucrose gradient centrifugation. Kinetics of the molecular weight increase in newly synthesized DNA were assayed after cells had been incubated in the presence or absence of caffeine. Two separate pathways of postreplication repair have been tentatively identified by mutants derived from four complementation groups. The proposed caffeine sensitive pathway CAS ; is defined by mutants which also disrupt meiosis. The second pathway CIS ; is caffeine insensitive and is not yet associated with meiotic functions.
