Toxicity, but neurological, hepatic, thyroid, and visual toxicities have also been of concern, and the minor gastrointestinal and skin reactions and insomnia have also been factors.24, 33 It is generally agreed that less problematic toxicity occurs when amiodarone is used as a first-line drug at a relatively low dosage and in patients who are not necessarily severely ill.33-36 Series in which toxicity is rather frequent and serious are likely to use amiodarone as an agent of late or last resort for patients who are often very ill, and consequently the dosages are rather high and treatment may be maintained even in the presence of some toxicity.37-39 In the present study, no patient died as a result of amiodarone toxicity. Possible pulmonary toxicity occurred in three patients 6.3% ; , although the link was not definitely established in two of them, and there was almost certainly no link in the third. Hepatic enzyme increase was equally common among amiodarone and placebo patients and always resolved on stopping the drug. Chemical hypothyroidism was common, occurring in 25% of amiodarone patients but in no placebo patient. Long-term follow-up of these patients will determine whether normal thyroid function resumes after completion of the course of amiodarone therapy. Proarrhythmia is a recognized hazard of most antiarrhythmic drugs, although it is probably rare with amiodarone therapy.24 The three patients who developed apparent proarrhythmia in the present study were found to be receiving placebo. Abnormalities in day vision occurred but were not attributable to amiodarone. Minor side effects were common, particularly during the loading phase. However, apart from skin manifestations, they were no more common among the amiodarone patients than among those receiving placebo, which emphasizes the importance of placebo controls for the objective assessment of adverse experiences. Although minor side effects often led to dosage modifications, they rarely resulted in permanent discontinuation of the study drug. Permanent early discontinuation for reasons other than death or resuscitated VF occurred in 17 of patients 35% ; in the amiodarone group and 10 of 29 patients 34% ; in the placebo group. These are similar to the rates observed previously in a study of antiplatelet drugs in patients with unstable angina40 and in a Swiss trial of amiodarone after MI.20 They are slightly higher than rates in trials of , B-blocker therapy after MI.41, 42 The pattern of discontinuation was such that approximately half of the amiodarone patients who eventually terminated early had received at least 1 year of treatment, indicating that they may have benefited from therapy during the period of highest risk after MI and, in addition, would have had detectable plasma levels of amiodarone and DEA for several months after drug discontinuation. Some of these species are important to nitrogen cycling and degradation of pollutants. Like bacteria, fungi are important for retaining nutrients in the soil. In addition, many of the secondary metabolites of fungi are organic acids, so they help increase the accumulation of humic-acid rich organic matter that is resistant to degradation, and may stay in the soil for hundreds of years. Soil fungi can be grouped into three general functional groups based on how they get their energy. Decomposers that convert dead organic material into fungal biomass, CO2 and small molecules, such as organic acids. Mutualists the mycorrhizal fungi colonize plant roots. In exchange for carbon from the plant, mycorrhizal fungi help solubolize phosphorus and bring soil nutrients to the plant. The third group of fungi, pathogens or parasites, cause reduced production or death when they colonize roots and other organisms. That's just the beginning of the roster of underground residents. Let me introduce you to the protozoa that eat bacteria and release the excess nitrogen to plants and other members of the underground community. Meet the nematodes, non-segmented worms that function at several levels of the soil food web. Let's not forget the many bugs, also known as arthropods, that make their home in the soil. They include insects, such as springtails, beetles, and ants; crustaceans such as sowbugs; arachnids such as spiders and mites; myriapods, such as centipedes and millipedes; and scorpions. Nearly every type of soil is home to many different arthropod species. Certain row-crop soils contain several dozen species of arthropods in a square mile several thousand different species may live in a square mile of forest soil. From this perspective, it becomes clear that the priority of conserving and restoring functional group diversity can exceed the need to conserve individual taxonomic diversity. One way to do this is by restoring soil biology. Compost is ideal for that purpose. The compost should have a huge functional group species diversity, not just bacteria but fungi, protozoa, nematodes and microarthropods, as well as organic matter for them to feed on. The compost should be made locally, so that its soil biology is similar to the soil on which it is applied. Also, compared to a field with a two-year crop rotation, a field with a four crops grown in rotation may have a greater variety of food sources and therefore is likely to have more types of bacteria, fungi, and other organisms. A cleanly-tilled field with few vegetated edges may have fewer habitats for arthropods than a field broken up by grassed waterways, terraces, or fence rows. It should be public knowledge that the soil and aboveground life are undivided entities. Elliot conducted a trial that lasted four to five years and compared two fields of similar soils but seeded to different mixtures. Field #1 used a simple mixture consisting of cocksfoot, perennial ryegrass, lateflowering red clover, S.100 white clover and one pound per acre of chicory, a total of 25 pounds of seed being sown per acre. Field #2 contained the same legumes and grasses as Field #1, but with the following additions: three pounds per acre of chicory, four pounds burnet, two pounds sheep's parsley, two pounds kidney vetch, one pound yarrow, two pounds lucerne, and two pounds American sweet clover, for a total of 45 pounds of seed per acre. Both fields achieved equal establishment, yet despite the variation of growth, which was deliberately varied for test purposes, whenever cows were led from Field #1 to Field #2 the herbal ley ; milk yields always increased. This was so even when cattle were removed from Field #1 with ample grazing available ; and moved to and taxol. Vitreous. No foreign bodies were found in any of the eyes. No canine eyes were diagnosed with comparable lesions in the same period. Discussion All eyes in this series had sustained penetrations ventrally in the 6 o'clock meridian along with posterior lens capsule rupture. The presence of two penetration sites allows the trajectory of the penetrating object to be determined with reasonable accuracy. It is only possible for an object to take this trajectory if the direction of travel is ventral to dorsal and therefore from the mouth via the soft palate unless it travels through bone. In Case 3 the bone quality was considered poor in the region of the carnassial tooth and this may have placed the cat at increased risk of penetration. All cases showed the same pattern of penetration and all had a history of recent dental treatment including molar extraction, except for Case 2 where there was an 11 month interval between dental treatment and the reported onset of clinical signs. The lack of a bony floor to the orbit leaves the cat vulnerable to ventral orbital penetration by foreign bodies and iatrogenically. This is a particular risk with dental extraction in the cat where there is only a thin rim of alveolar bone surrounding the last upper molar caudally. Good dental technique will minimise the risk. Only steady gentle pressure should be applied with dental elevators avoiding excessive force. The opposite hand can be used to cradle the working surface and act as a "stop" if the instrument should slip Smith et al, 2003 ; . It is difficult to gauge the frequency of the injury described here as there are so few reports of documented cases in the literature. The authors speculate, however, that the number of cases in the literature under-represents the scale of the problem and suggest two possible reasons. Firstly, the connection between the dental treatment and the ocular changes may not be made by the clinician or the owner. Secondly, when such eyes are enucleated they may not be submitted for pathological analysis which would confirm the diagnosis. This series is of enucleated eyes only and cannot indicate the prognosis for management of eyes which have sustained this injury. Lens rupture, phacoclastic uveitis and intraocular infection are always, however, potentially serious problems and the pathological findings in these eyes showed that the decision to remove the eyes was fully justified in the circumstances.
